The Geriatric Care management program at Jewish Family Service, Inc. of Broward County was created to help close the gap between families living locally or out-of-state and their older adult relatives who reside in Broward County, Florida.

Our goal is to work together with families to enhance the older person’s ability to live his or her remaining years with dignity and self respect through the support of a Care Manager. Jewish Family Service offers reassurance to families by keeping the family involved in all aspects of the process. Our team of professional Care Managers is the direct link between the senior and the family, helping to prevent minor problems from becoming major crises.

This Geriatric Care Management Program provides an in-depth assessment and evaluation of the senior, including the development of a care plan. This plan includes appropriate recommendations for services to help deal with changes related to the aging process. Safety, dignity and high quality of life are the long term goals of the care plan. Monitoring, along with the careful management of local resources, assures families that our care plan stays in place.
